Practice Statement of Purpose
Our objective is to ensure that Botesdale Health Centre provides people with safe, effective, compassionate, high quality care.
Our aims are to work in the following ways:
- To work in partnership with our patients, their families and carers and to involve them in decision making about their treatment and care;
- To treat patients and staff fairly, respectfully and without discrimination;
- To use NHS resources efficiently and effectively whilst providing clinically appropriate treatment;
- To focus on prevention of disease by promoting good health and wellbeing and offering care, advice and support to our patients;
- To provide a safe, clean and suitably equipped environment for patients and staff;
- To work within local and national governance, guidance and regulations;
- To involve patients in the development and maintenance of services through the patient participation group and other patient feedback channels;
- Listening to and acting upon patient experiences;
- To ensure that our services are audited and continually improving
We were inspected by CQC on 26th July 2019. The CQC inspectors looked at all of the objectives and aims above and found us to be ‘Good’ in all areas. You can read the full report via the hyperlink below.

This Practice will provide those Essential Services as outlined in our General Medical Services Contract. • To ensure the management of patients who are ill or believe themselves to be ill, from conditions from which recovery is generally expected, for the duration of that condition, including relevant health promotion advice and referral as appropriate, reflecting patient choice wherever practicable. • To provide management of patients who are terminally ill. • To provide management of chronic disease in the manner determined by the practice, in discussion with the patient and in line with the evidence based practice. • To provide a high quality general medical service which is monitored and audited to ensure it meets the needs of our practice population. • To ensure that provision of our services is in a safe, clean and effective environment. • To ensure that our services users receive advice and treatment which affords them privacy, dignity, choice and information to manage their condition. • To ensure our equipment, instruments and facilities meet with infection control guidelines. • To establish, build and maintain an effective communication forum with our service users in order to listen and, where practicable, adapt their needs. • To provide a transparent complaints service that listens to our service users in order to listen, and where practicable, adapt to their needs. • To manage our service provision effectively to allow for responsive action with regards for capacity and demand for services. • To recruit staff appropriately in line with the guidance and regulation. • To ensure our staff are trained to create an effective workforce and so meet the expectations of our service users. • To manage and maintain the workforce effectively. • To ensure equality and diversity in the workplace through training. • To ensure a high level of clinical governance through audit and training. • To ensure a high level of information governance through audit and training.
LOCATION, SERVICE USERS AND REGULATED ACTIVITIES: All regulated activities are undertaken at: Botesdale Health Centre, Back Hills, Botesdale, Diss, Suffolk, IP22 1DW and are under management of the registered manager Dr D MacEachern.
Our building has wheelchair access and limited parking which is at the front of the building. A language translation service is available by telephone. Home visits are provided for those patients who are unable to attend the appointment due to a medically appropriate reason or physical impairment. The practice has two floors and a lift. Our regulated activities are: • Diagnostic and Screening Procedures Cervical Screening, blood pressure monitoring, cardiovascular risk assessment, ECG, Spirometry, referral for other screening as appropriate. • Family Planning Services Conception, Contraception. • Maternity and Midwifery Services Ante natal referral and screening in conjunction with community midwifery service. • Surgical Procedures Minor operations appropriate to a primary care setting. • Treatment of disease, disorder or injury Consultation and appropriate referral to manage everyday illnesses, disorders or injury, preventative medicine to enhance quality of life, Chronic disease management for patients with a diagnosed long term condition. Service Users Our service users are our registered population which spans any age, ethnicity or gender.
