Who can see my Health Record?

To treat you safely and well, it is important that professionals you see can access your health record. Only by letting your GP surgery know it is ok will your notes be available to be seen by other professionals such as hospital clinicians, paramedics or district nurses. 

How do I make my record available to health professionals? Simple. Complete this form and hand it into your GP surgery. There are two ways that this sharing can happen and it’s important you understand what they are so you can make the right decision for you. Patient Online Access allows you to have online access to your own GP Medical Record, please see below for more information.

Please be aware that we may use new providers or suppliers to help us quickly adapt during the COVID19 outbreak and continue your care effectively. For example, we may use a new provider for video consultations. We may not be able to add these to our transparency materials right away, we apologise for this but please be assured all our processors are bound by contract to protect to your data.

Patient Online Access


Patient Access allows our Patients to access services online to Manage Appointments, Order Repeat Presciptions and more recently the ability to view parts of their Medical Record - Medication, Allergies and Adverse Reactions.


To request this online access you will need to come into the Practice and ask for an application form complete from Reception, or download a copy here and return it to the Practice.

What is a Summary Care Record (SCR)?

An SCR is an electronic record of important patient information, created from GP medical records. It can be seen and used by authorised staff in other areas of the health and care system involved in the patient's direct care.

The information that the SCR holds includes, the medication you take, the allergies you have and any medication that youve had adverse reactions to.

There might be an occassion where you need to see a doctor or nurse who does not know you. If they do not know about your care, your SCR could:

  • Stop them making a mistake, because they can see your medicines, allergies or what medicines make you ill.
  • Help them see your information straight away on a computer.

The medical staff treating you will ask for your permission, so they can look at your SCR to help them treat you quickly and safely.

SCR with Additional Information
You can also choose to have other useful information added to your SCR, including:

  • Your illnesses and any health problems.
  • Operations and vaccinations you have had in the past.
  • How you would like to be treated – such as where you would prefer to receive care. 
  • What support you might need.
  • Who should be contacted for more information about you.

Summary Care Records for Children
f you are the parent or guardian of a child under 16 and believe they may benefit from additional information in their SCR, you can discuss this with your GP practice.

Vulnerable Patients and Carers
Certain vulnerable patient groups such as frail elderly people or those with detailed and complex health problems can particularly benefit from additional information in their SCR. If you are a carer for another person and believe that they may benefit from additional information in their SCR, then you can discuss this with them and their GP practice.

Your Full Electronic Health Record

Your full electronic health record is held by your GP surgery. It can also be made available to health and social care staff, if they are involved in your direct care. Staff must still ask for your permission before they look at your record. This also allows your surgery to see what other staff are doing to support and treat you. If there are certain parts of your record that you wish to keep private, your surgery can do this.

Does this mean anyone can just look at my record? No. Your record can only be seen by staff who are currently involved in your direct care, have a need to see it, and have asked for your permission. The only exception to this is in case of an emergency. For instance, if you were taken to hospital unconscious, a doctor could look at your record without your permission. If this happens, a permanent alert is created showing who looked at the record and why.

Are you going to sell the information in my record? Never. If your record is shared, it’s only ever available to staff for the purposes of your care, nothing else.

Can I change my mind? Yes. Just tell your surgery and they can update your decision at any time.
